What are the print options you selected and was there any error logged by the Agfa printer?

These look like the Agfa printer settings. What about the print parameters you selected from PacsOne Server when printing to this printer (they are similar but not quite the same)?

1. Did you check if the Agfa printer has logged any error that correspond to the print job from PacsOne?

2. You have selected the 2r x 2c display format for the output film, so how many images were you trying to print?

3. Have you tried deleting the customized printer profile for PacsOne and simply use the default profile instead?

Did you try deleting the GENERIC-GENERIC host profile configured for PacsOne on the Agfa printer, then print a sample image to see if it looks Ok? i.e., from PacsOne Server, print a sample image to Agfa as the default (no specific model defined) printer, and on Agfa no custom profile is defined for PacsOne so that all default settings are used on both ends.

1. Did you check with the Agfa technical support on the black image?

2. Did you try printing a different modality image from PacsOne to the same Agfa printer and see if the same problem exists or not?
